Buy any 2 Andes 4.67 oz bozes ( any flavor) between 8/1/11 and 12/31/11
Mail in the completed rebate form, UPC bar codes from 2 boxes, and the original cash register receipt with the price you paid circled, dated between 8/1/11-12/31/11

Do you happen to still have the rebate form or a copy of it? I am asking because I can't find my copy. Does it state on the requirements you must have the ORIGINAL upc? I do remember it stated you needed the original cash register receipt but not the originial UPC. My rebate was denied because I did not have the original UPC I just sent a copy and now they are loooong gone. -Lori
